Conditions in the places where people live, learn, work, and play affect a wide range of health risks and outcomes. To help address social determinants of health, the American Academy of Family Physician’s EveryONE Project offers tools for screening patients to identify social needs, and addressing health equity in health care sites. The Neighborhood Navigator provides the next step for improving social determinants of health among patients by linking them to thousands of social services in their area that can provide help with child care, housing, transportation, employment and legal services.
This interactive tool connects patients to resources and programs in their neighborhoods. It includes over 40,000 social services by zip code, including:
- Food
- Housing
- Transportation
- Employment aid
- Legal aid
- Financial aid
Patients may also seek services on their own when referred to the patient-facing Neighborhood Navigator available on familydoctor.org.
