After nearly 18 months of piloting an embedded FoodShare outreach specialist model at Midtown Clinic, the food security initative has gained valuable insights into how families prefer to connect with food, nutrition, health insurance and other community supports.
Navigators in action
The FoodShare navigator spends one day per week at the clinic, participating in team huddles and working with staff to develop resources and processes that enable the team to assist families even when she is not onsite.
A second navigator from NourishMKE is four months into piloting a shared service model at Forest Home Clinic, where a similar role supports families one day per week. Having spent a longer period at Midtown Clinic, we now have more experience and data to understand the model’s impact on families and can use those learned lessons to apply to another clinic.

The FoodShare Outreach Specialist is just one of several strategies that the FoodCare Coalition is working on to increase access to FoodShare and nutritional resources.
